These standards are based on My Shared Pathway CQUIN, which was developed to ensure service users moved through the pathway quicker and more seamlessly. This looks at ensuring a clear pathway that is planned on or before admission, and through to the point of discharge. It also ensures that people are involved in their care and treatment and that there are achievable outcomes that inform service users about what to achieve in order to progress to the next step. It also aims to reduce repetition when service users move within and between services, and lets service users know what to expect and when and how their care and treatment will be reviewed.
